All I need for medico-legal reasons is your name, address, phone number, name of
registered doctor and current medication. However, the history of accidents and injuries,
dental work, eyesight and family medical history can also be relevant. For cranial
work, birth details, and anything you know about your mother’s pregnancy with you
might also have a bearing. However, you might also find the form (which will not
be an ‘official’ record) doesn’t allow you to tell your ‘story’, in which case just
use it to make your own more realistic notes. Whatever you are able to do, it will
save us some time at your first session.
If you have an interest in crystals, astrology (and maybe have your own birth chart),
ayurvedic healing, acupuncture/Chinese medicine, Buddhism, past lives, shamanism
and so on, these and many other topics can also be included.
